You must be under a doctor’s care to have eligibility for Banner Home Care service. Your doctor is responsible for building and reviewing your individual care plan. It is also required to have a doctor certify that you are home bound with a need for skilled nursing care, physical therapy, speech-language pathology services or occupational therapy.
